Metallic explants resulting from orthopedic surgical therapeutic completion are classified as solid waste with potential for contamination. Objective: To analyze in the literature how environmental management occurs in hospitals through the disposal of waste from metal orthopedic explants. Methodology: This was an applied, practical, quantitative, experimental, exploratory, empirical and transversal research with the investigative objective of confirming the hypotheses. The research was carried out in all Brazilian States, in public and private hospitals, considered as a reference for the treatment of orthopedics and traumatology. Three distinct groups were compared: Brazilian hospitals certified by ISO 14001, hospitals with ONA accreditation with a level of excellence and those not certified, using the inductive method. Main Results: The explants must be registered and controlled for recycling; a step contemplated in the health waste management plan (PGRSS). However, final disposal is a wide problem due to economic issues creating barriers to the application of laws, contributing to environmental deterioration. When it comes to recycling surgical explants, stainless steel can be fully reused. Main conclusions: In Brazil, there are no exact statistics regarding the number of generators or the amount of waste from health services and the explants are discarded as sharp piercing waste.
